Job Description

Position Title: Priest Wellness Coordinator
Classification: Nonexempt, part-time (16-20 hours/week)
Department: Priest Wellness
Reports to: Vicar General

Position Objective
To assist priests, with the primary focus on retired priests, in achieving an optimal state of wellness by performing tasks relative to the management of their health and providing professional coordinated services.

Essential Duties
Work in collaboration with the diocese, priest, priest’s family members, physicians and other health care personnel in priest health evaluation and treatment.
 Assist in scheduling priests’ health care appointments and provide transportation to appointments as needed.
 Serve as the priest’s advocate and personal representative during hospitalizations and medical appointments as needed.
 Complete and maintain health care records for each priest.
 Ensure that all priests have documented their end-of-life care instructions and copies are on file in the Chancery office.
 Maintain an open line of communication between priest, wellness coordinator and Vicar General and provides updates on priest health status as needed.
 Maintain awareness of priests’ condition and changes in condition as needed.

Coordinates priest health insurance coverage and benefits with Human Resources Department.
 Verify insurance coverage per procedure or hospitalization.
 Assist priests with reimbursement of copay expenses.
 Assist priests with enrollment in Medicare/Medicaid.

Assists priests with personal and environmental difficulties, which predispose illness or interfere with optimal mental and physical health.
 Educate priests, their families and their Durable Power of Attorney designees through individual or group conferences; to understand, accept, and follow medical recommendations and the interdisciplinary plan of care based on the individual needs of the priests.
 Coordinate and utilize resources to assist the priest with returning to their optimal level of health and resuming life in the community or learning to live within the limits of their disability.
 Assist in determining best home environment for the priest based on current health condition and coordinate transition to assisted living, nursing home or other long-term care facility.

Knowledge, Skills and Abilities
 Excellent written and oral communication and interpersonal skills.
 Ability to maintain confidential documents and information; knowledge of HIPAA rules and regulations.
 Working knowledge of pharmacology and medical terminology.
 Understands insurance guidelines, federal and state health programs, standards of care, and reimbursement rules and regulations.
 Understands organizational and community resource management processes and continuum of care.
 Understands human disease processes and utilizes advanced critical thinking skills in complex clinical analysis, priority management and decision making.
 Able to work independently.
 Keeps commitments made to others. Does the right thing—even when it’s difficult.
 Watches/listens to verbal and non-verbal cues that create a deeper understanding of the message.
 Shows interest in, anticipates and responds in a timely manner to needs.
 Goes beyond basic service expectations to help implement complete solutions.
 Seeks feedback to ensure needs have been fully met.
Basic Qualifications
• RN degree required, BSN preferred.
• 5 years acute care of adult and geriatric clients.
• 1 year management experience.
• Proficiency in MS Office, e-mail, and internet navigation.
• Valid Driver’s License
